Adam Brunskill was among four workers on the roof at Bestway Wholesale in Bloxwich Lane near Reedswood when he fell through an unprotected glass-wire skylight to the concrete floor on July 14, 2020. The labourer suffered a severe brain injury as a result and died in hospital the following day.
The team were employed by Wayne Clarey Roofing & Cladding Limited and were beginning a 10 to 12-week task of over-cladding a large industrial unit at the site near the M6 at junction 10 with principal contractor Proclad Developments.
Mr Brunskill, a first team rugby player at Bishop Auckland Rugby Club, had received no accredited training prior to starting the job two days earlier and was allocated the task of laying down bars on the roof for the over cladding when it was discovered that he was absent at about 9am.
An ensuing investigation by the Health and Safety Executive (HSE) found that it was a "very preventable accident" and that the skylights could have been covered or safety netting installed below to protect the employees due to the risk involved in working at height.
